EXECUTIVE LOCKER LAUNDRY PROGRAM
Enrollment Agreement & Liability Waiver
By enrolling in the Club 245 Executive Locker Laundry Program, I acknowledge and agree to the following
terms:
1. Laundry Services: The program includes five (5) washes of gym clothing only. Streetwear, business attire,
dress clothing, and other non-athletic garments are strictly prohibited.
2. Drop-Off & Return: Laundry bags placed in the towel bin in the locker room will be returned to my
executive locker within forty-eight (48) hours of drop-off.
3. Garment Responsibility: I understand that all garments are submitted at my own risk. Club 245 and Body
By Mark Wellness LLC are not responsible for normal wear and tear, shrinkage, fading, discoloration, or
damage resulting from laundering, including damage to delicate or improperly labeled garments.
4. Lost Items & Compensation: In the event that laundry is lost due to the mishandling of Club 245 or its
representatives, compensation shall be limited to a maximum of $80 per affected laundry bag, regardless of
the number or value of items contained therein. Compensation will be issued exclusively as a credit toward
my Club 245 membership, not as cash or a refund.
5. Release of Liability: To the fullest extent permitted by New York law, I hereby release and hold harmless
Club 245, Body By Mark Wellness LLC, and their respective owners, employees, agents, and representatives
from any claims, losses, or damages arising from participation in this program, except as expressly provided
above or where liability cannot legally be waived.
6. Acknowledgment: I confirm that I have read, understand, and voluntarily agree to these terms and
conditions.
